The patient is a 26-year-old year male from Porto Novo in Tamil Nadu, India. He began noticing the development of pain and swelling in his left upper jaw a few months ago. This has now increased to the point where there is severe pain and discomfort in that region. There is also a feeling of heaviness in that region.
Examination at a local dental clinic for diagnosis of his problem
He had presented at a local dental clinic where imaging studies had been obtained. This revealed the presence of an impacted molar in the left maxillary sinus. Suspecting this to be a dentigerous cyst, the dentist had immediately referred the patient to our hospital for management. The patient gives a history of undergoing extraction of his left upper second molar a few months ago.

Treatments ranging from simple extractions to complicated craniofacial surgeries are routinely performed at our hospital. Facial trauma surgery and jaw reconstruction surgery are specialty surgeries performed here.

Tooth extractions can be the best option when teeth become damaged by decay or break down due to trauma or wear, they can sometimes be restored with a filling, root canal, or crown. When the damage is too great, your dentist may decide that the best course of action is to have the tooth removed, a dental extraction. If this is the case, your dentist will develop a plan for extraction, taking into consideration your age, health, and the overall condition of your mouth.

More and more people are getting dental implants to replace missing teeth. They're a long-term solution that is imbedded in your jawbone, just like your natural teeth. They even go your natural teeth one better, since they can't develop cavities. Plus, unlike fixed bridges or removable dentures, dental implants will not affect neighboring healthy teeth or lead to bone loss in the jaw. If properly cared for, dental implants can last a lifetime.

The oral and maxillofacial surgeons at University of Illinois at Chicago (UIC) College of Dentistry have the specialized education and training in the complexities of the bone, skin, muscles and nerves involved, to ensure you get the best possible results.

Chain and Bracket, Surgical Exposure of Impacted Tooth For Orthodontics

Although we often think of the third molars, or "wisdom teeth," when we think of impacted teeth, any of the permanent teeth can fail to erupt properly. This lack of normal tooth eruption can be due to a variety of factors such as failure of the developing tooth follicle to have the correct orientation, improper developmental timing of the tooth bud, or ankylosis of the developing adult tooth to the surrounding alveolar bone. The wisdom teeth can just be extracted and removed, but other teeth that are impacted will have a more noticeable effect on the smile's appearance and also on a patient's occlusion and function.

This is especially true of the upper canines, which are the last of the front teeth to emerge and sometimes do become impacted. Usually, they are completely obscured by the bone and visible only in x-rays.

Patients who are facing this issue can benefit from a collaborative treatment between an oral surgeon and an orthodontist. The oral and maxillofacial surgeon performs a procedure to uncover the impacted tooth, and the orthodontist moves it into place using specific appliances.

This treatment is known as a surgical tooth uncovering and chain-and-bracket procedure. The orthodontist begins the course of treatment, attaching the orthodontic appliances to the visible teeth, creating room for the unerupted tooth. Then, the patient is referred to the oral surgeon for surgical exposure of the impacted tooth and placement of an orthodontic appliance to facilitate its eruption. Sometimes the maxillofacial surgeon may need to also luxate or mobilize the tooth as well, which makes the movement required easier for the orthodontics to achieve in a more efficient manner. That surgery involves lifting the gum tissue overlying the unerupted tooth and removing any bone that may obstruct it from being moved into place. A bracket and chain are bonded to the tooth, and the chain is later connected to the rest of the braces. When everything is in place on the tooth, the gum tissue is then repositioned and sutured. This is all performed under the comfort of sedation and typically can be completed within an hour.

After taking a few weeks to heal from the procedure, patients return to the orthodontist to get the adjustments that will put tension and rotation on the tooth in order to bring it properly into position. This process may require considerable patience, as it can take up to a year for everything to be complete.

Scaling and root planing are different forms of treatment for periodontitis periodontitis (or pyorrhea or gum disease).

In this 3D animation from Clínica Médico Dental Pardiñas (http://j.mp/cPardinas) we talk about the diagnosis and treatment of periodontitis (pyorrhea) by performing scaling and root planing.

It shows how plaque and tartar will accumulate around the teeth and how it is removed using different instruments, thus returning the gum to a healthy state.
Here we can see the process of probing, one of the elements used to diagnose periodontal disease. The greater the measure, the greater is the existing periodontal pocket.
The progressive accumulation of plaque and tartar causes that the resorption of the bone that supports the teeth. Gums will show signs of inflammation and bleeding, among others. Once the accumulated plaque mineralizes, it becomes tartar, which can no longer be removed with regular brushing.
To remove tartar, curettes and ultrasound devices that are introduced below the gum are used. Polishing tools can also be used to polish the tooth surface. The gingiva in the absence of plaque and tartar will no longer be inflamed and will return to a healthy state.
This can originate retractions.
In a side view we can see the same process of scaling and root planing and the posterior healing of the gums.
