After performing a dental extraction, the place on which the roots of the tooth are settled, called the alveolus, is exposed, forming a blood clot on it, thus generating the first stages of healing and protecting the nerve endings of the empty dental socket.
00:21 What does dry socket look like:
Occasionally, this clot may not form due to various factors, or it may disappear due to contamination by food debris or smoking during this healing phase. This produces a process called dry socket. When the nerve endings are exposed, intense pain is generated in the area, which usually appears during the first 3 days after a tooth extraction.
00:48 Dry socket treatment:
The main treatment is symptomatic with pain medication. It is also important to remove food debris by irrigating the area with saline solution. Curetting of the socket may be indicated to decontaminate the bone surface and cause bleeding for a new clot to form.
01:27 Treatment with Plasma Rich in Growth Factors:
Another effective treatment is the placement in the alveolus of plasma rich in growth factors, since it has bacteriostatic properties and favors tissue regeneration. It can also be used preventively, placing it at the time of extraction.

Chain and Bracket, Surgical Exposure of Impacted Tooth For Orthodontics
Although we often think of the third molars, or "wisdom teeth," when we think of impacted teeth, any of the permanent teeth can fail to erupt properly. This lack of normal tooth eruption can be due to a variety of factors such as failure of the developing tooth follicle to have the correct orientation, improper developmental timing of the tooth bud, or ankylosis of the developing adult tooth to the surrounding alveolar bone. The wisdom teeth can just be extracted and removed, but other teeth that are impacted will have a more noticeable effect on the smile's appearance and also on a patient's occlusion and function.
This is especially true of the upper canines, which are the last of the front teeth to emerge and sometimes do become impacted. Usually, they are completely obscured by the bone and visible only in x-rays.
Patients who are facing this issue can benefit from a collaborative treatment between an oral surgeon and an orthodontist. The oral and maxillofacial surgeon performs a procedure to uncover the impacted tooth, and the orthodontist moves it into place using specific appliances.
This treatment is known as a surgical tooth uncovering and chain-and-bracket procedure. The orthodontist begins the course of treatment, attaching the orthodontic appliances to the visible teeth, creating room for the unerupted tooth. Then, the patient is referred to the oral surgeon for surgical exposure of the impacted tooth and placement of an orthodontic appliance to facilitate its eruption. Sometimes the maxillofacial surgeon may need to also luxate or mobilize the tooth as well, which makes the movement required easier for the orthodontics to achieve in a more efficient manner. That surgery involves lifting the gum tissue overlying the unerupted tooth and removing any bone that may obstruct it from being moved into place. A bracket and chain are bonded to the tooth, and the chain is later connected to the rest of the braces. When everything is in place on the tooth, the gum tissue is then repositioned and sutured. This is all performed under the comfort of sedation and typically can be completed within an hour.
After taking a few weeks to heal from the procedure, patients return to the orthodontist to get the adjustments that will put tension and rotation on the tooth in order to bring it properly into position. This process may require considerable patience, as it can take up to a year for everything to be complete.

The patient is a 25-year-old female from Kumbakonam in Tamil Nadu, India. She had developed severe pain and swelling on the left side of her mandible. The patient had a grossly decayed left third molar. There was also severe trismus with limited mouth opening due to the presence of the infected tooth.
The patient had presented at a nearby dental clinic for management of her problem. An OPG had been obtained, which revealed bilaterally impacted mandibular third molars. The left third molar was grossly infected. The right third molar was completely submerged within bone and the inferior alveolar nerve was passing through the tooth.
Upon viewing this, the doctor had realized that this was a complicated extraction that needed to be performed by an experienced oral and maxillofacial surgeon. He had subsequently referred the patient to our hospital for extraction of her impacted mandibular molars.

The apicoectomy is a procedure to remove an infection that affects the root of a tooth and the adjacent tissues. It is performed in cases where the endodontic treatment has failed, presence of canals or false canals unsealed or in teeth where the access to the apex is not possible by a conventional root canal treatment.
00:40 How is an apicoectomy performed?
To do so, an incision is performed, and sufficient bone is removed to get access to the root apex. Then the infectious cyst is removed cleaning and irrigating the area. A beveled shape cut is made to the root removing the apex and sealing the root with different materials. The area is sutured and will regenerate until its complete healing.

Yes, he can. A patient is classified as a diabetic if the fasting blood glucose is constantly above 140 mg/dl. A fasting blood glucose level of below 200 mg/dl is considered safe for most minor oral surgical procedures including extractions. However being diabetic makes you prone for lot of infections and has a poor effect on the healing capacity of the whole body. Hence it is mandatory to have an adequate amount of broad spectrum antibiotic coverage before and after your dental extraction. Also an optimum levels of blood glucose levels has to be maintained. So don’t skip your insulin or any other oral antihypoglycemic medications prescribed by your doctor. A preoperative blood glucose evaluation will be required by your dentist. Also if possible, a morning appointment schedule for your dentist would be ideal. Being diabetic doesn’t exclude you form any kind of dental treatment or you have to shy away from going to your dentist. You can always get your regular checkups doe, your scaling and restorations and fillings done without any hassle but for slightly complex and invasive procedures like the gingival flap surgeries or the implant procedures, there are few things you have to keep in mind and keep in check, the first is the random blood sugar, the RBS test will be required. The second one is HbA1C below 7 is what is ideal and the third and the key element is to be in constant touch with your dentist. Keep coming for follow up’s and schedule appointments and request the dentist for every moth appointments and they would gladly oblige and if anything goes wrong, we would be glad to help you out and monitor it.
