Do you wish your third molars, or "wisdom teeth," never came in? Many of us do because they often cause problems and need to be removed. Fortunately, a fairly routine procedure, wisdom tooth extraction can be the answer for an impacted tooth, which fails to pass through the gums, or an erupted tooth, which does pass through the gums, but may still need to be removed. During this interactive procedure, you will begin by taking x-rays of the tooth in question, numbing the site, and then injecting the anesthetic. Then you will loosen the periodontal ligament and use a dental elevator to lift the tooth. Finally, you will use dental forceps to loosen and remove the tooth and leave your patient to recover. Scaling and root planing are different forms of treatment for periodontitis periodontitis (or pyorrhea or gum disease).

It shows how plaque and tartar will accumulate around the teeth and how it is removed using different instruments, thus returning the gum to a healthy state.
Here we can see the process of probing, one of the elements used to diagnose periodontal disease. The greater the measure, the greater is the existing periodontal pocket.
The progressive accumulation of plaque and tartar causes that the resorption of the bone that supports the teeth. Gums will show signs of inflammation and bleeding, among others. Once the accumulated plaque mineralizes, it becomes tartar, which can no longer be removed with regular brushing.
To remove tartar, curettes and ultrasound devices that are introduced below the gum are used. Polishing tools can also be used to polish the tooth surface. The gingiva in the absence of plaque and tartar will no longer be inflamed and will return to a healthy state.
This can originate retractions.
In a side view we can see the same process of scaling and root planing and the posterior healing of the gums.

Infected or broken teeth often cannot be repaired and must be removed. If there is enough bone in the jaw, a dental implant can be used to replace the removed tooth. This patient came to me with a large swelling and pain from her lower right first mandibular molar. We determined that the cause of the problem was that this tooth most likely had a vertical root fracture. This then led to the death of this tooth, and subsequent infection and pain.

In cases like these, the only treatment is for us to remove the tooth.

To extract the tooth, it is more simple to section the tooth into different pieces. That way, the tooth is less likely to break when we try and remove it.

In this video, I discuss what the problem was, our surgical plan, and then walk through how we did the procedure from start to finish.

This horse was featured on a previous video. The tooth had fractured secondary to dental decay. The tooth had fragmented and had been partially removed. Instead of subjecting the horse to a general anaesthetic, which carries a small but significant risk, it was sedated using a morphine drip. The alveolar socket was anaesthetised by placing a maxillary nerve block and a gingival block. The tooth root fragment & alveolar socket was examined using an oroscope. This was also used to facilitate sectioning of the tooth root using an 3mm bone cutting bur. This was only partially successful as only several small fragments were removed. The decision was made to repulse the remaining tooth root fragment. Local anaesthetic was infiltrated under the skin in a position over the tooth root. A Coomer bone drill was used to drill a hole in the caudal maxillary sinus. A blunt ended trocar pin was placed over the tooth root and a mallet was used to drive the fragment through the alveolar socket. Radiograph were taken of the molar arcade revealing two further fragments (cementomas) located in the alveolar socket. These were relatively easily removed under oroscopic guidance. The resultant wound and sinus was flushed using sterile saline.The skin wound was closed with staples. The surgery had left hole (fistula) between the oral cavity (mouth) and sinus. This was packed with dental putty to prevent a sinusitis occurring due to food contamination from the mouth. These wounds can take up to 6 months to heal and need repacking regularly with dental putty.
