Extraction of a vertically impacted lower wisdom tooth with an elevator only. Do not apply the elevator between the second molar and third molar to avoid damage to the distal of second molar during the extraction of the third molar. The hardest bone and the best place for fulcrum for the elevator is on the buccal or distobuccal of the third molar crown.

The extraction of the mandibular third molar is the most frequent intervention in oral surgery and it is sometimes associated with less or more severe complications.
The orthopantomogram (OPG) is the golden standard for pre-operative diagnosis.
