This video shows a an apicoectomy surgical procedure step y step.
Apicoectomy is a dental procedure used to eliminate an infection in the root of a tooth and adjacent tissues. It is carried out in those cases in which a root canal treatment has not been effective.
The intervention is performed through the gum to access and cut the tip of the affected tooth, surgically removing the cyst and sealing the root with different materials. To finish, the area is sutured and over time the healing is completed

#apicoectomy #surgery
This patient came to me with a large swelling and pain from her lower right first mandibular molar. We determined that the cause of the problem was that this tooth most likely had a vertical root fracture. This then led to the death of this tooth, and subsequent infection and pain.
In cases like these, the only treatment is for us to remove the tooth.
To extract the tooth, it is more simple to section the tooth into different pieces. That way, the tooth is less likely to break when we try and remove it.
In this video, I discuss what the problem was, our surgical plan, and then walk through how we did the procedure from start to finish.

Some procedures do not require full anesthesia. In these cases, sedation is used to keep you comfortable and relaxed during your procedure. You may be aware or have memories of a procedure done under sedation, and this is normal. In some cases, it is necessary to change from sedation to full general anesthesia.
After your procedure, you will be taken to the recovery area where a nurse will monitor you as you continue to emerge from anesthesia. Do not make any important decisions for at least 24 hours after any type of anesthesia.

Horizontally impacted wisdom tooth removal http://www.sdcdentist.com An oral and maxillofacial surgeon or your dentist can remove (extract) a wisdom tooth. The procedure often can be done in the dentist's or surgeon's office. You may have the surgery in the hospital, especially if you are having all your wisdom teeth pulled at one time or if you are at high risk for complications.
If you have any infections, surgery will usually be delayed until the infection has cleared up. Your doctor or dentist may have you take antibiotics to help heal the infection.
Before removing a wisdom tooth, your dentist will give you a local anesthetic to numb the area where the tooth will be removed. A general anesthetic may be used, especially if several or all of your wisdom teeth will be removed at the same time. A general anesthetic prevents pain in the whole body and will cause you to sleep through the procedure. Your dentist will probably recommend that you don't eat or drink after midnight on the night before surgery so that you are prepared for the anesthetic.
To remove the wisdom tooth, your dentist will open up the gum tissue over the tooth and take out any bone that is covering the tooth. He or she will separate the tissue connecting the tooth to the bone and then remove the tooth. Sometimes the dentist will cut the tooth into smaller pieces to make it easier to remove.
After the tooth is removed, you may need stitches. Some stitches dissolve over time and some have to be removed after a few days. Your dentist will tell you whether your stitches need to be removed. A folded cotton gauze pad placed over the wound will help stop the bleeding.

This horse was featured on a previous video. The tooth had fractured secondary to dental decay. The tooth had fragmented and had been partially removed. Instead of subjecting the horse to a general anaesthetic, which carries a small but significant risk, it was sedated using a morphine drip. The alveolar socket was anaesthetised by placing a maxillary nerve block and a gingival block. The tooth root fragment & alveolar socket was examined using an oroscope. This was also used to facilitate sectioning of the tooth root using an 3mm bone cutting bur. This was only partially successful as only several small fragments were removed. The decision was made to repulse the remaining tooth root fragment. Local anaesthetic was infiltrated under the skin in a position over the tooth root. A Coomer bone drill was used to drill a hole in the caudal maxillary sinus. A blunt ended trocar pin was placed over the tooth root and a mallet was used to drive the fragment through the alveolar socket. Radiograph were taken of the molar arcade revealing two further fragments (cementomas) located in the alveolar socket. These were relatively easily removed under oroscopic guidance. The resultant wound and sinus was flushed using sterile saline. The skin wound was closed with staples. The surgery had left hole (fistula) between the oral cavity (mouth) and sinus. This was packed with dental putty to prevent a sinusitis occurring due to food contamination from the mouth. These wounds can take up to 6 months to heal and need repacking regularly with dental putty.
